1·Get an eye exam and/or contact lens fitting.

2·Purchase the required boxes of contact lens products as indicated between January 1, 2017 and December 31, 2017.

3·Go to www.rebate.alconchoice.com and select your contact lens brand to receive your Rebate Code.

4·Complete rebate submission process on www.rebate.alconchoice.com and electronically upload or mail the following required documents (you will receive the mailing address when you begin the rebate submission process on myalconchoice.com):

a) A copy of the original sales receipt with the eligible contact lenses circled
b) A copy of one original UPC code from one box of eligible contact lenses
c) A copy of your eye exam recipt and/or lens fitting receipt
d) (if submitting by mail): Your completed official Alcon Rebate form

If you don't have access to a computer, call 1-855-785-6629 to obtain your rebate. This will add two weeks to processing time.

Valid on purchases from January 1, 2017 through December 31, 2017. Rebate documents must be electronically submitted or mailed within 60 days of your contact lens purchase.

We recommend you make photocopies of your entire submission for your records. If you do not have access to the internet, or would like assistance with your rebate, please call 855-785-6629. This will add 2 weeks to processing time.

Terms and Conditions. Offer valid only on eligible purchases made between January 1, 2017 and December 31, 2017. Product purchases must be made within 90 days of contact lens fitting/exam. Rebate submission must be mailed in a stamped envelope to the address listed on the Alcon Official Rebate Form and mailed to this address within 30 days of your contact lens purchase. Only one rebate submission per envelope. Offer is limited to one fit rebate with annual supply quantity purchase per valid promotional period except for valid combinations as outlined in the qualifying products chart. Allow 6-8 weeks for delivery. No P.O. Boxes (except for residents of North Dakota and where required by law), only street and rural addresses are acceptable. Fraudulent submissions could result in prosecution. Not responsible for lost, late, damaged, illegible, undelivered, or postage-due rebate requests, which will not be honored.
