Here’s How To Receive Your Rebate

Offer valid only for purchases made in the U.S. by U.S. residents.



1·Get a contact lens fitting and/or eye exam.

2·Purchase required number of boxes as indicated below:

3·Go to www.myalconlensrebates.com and enter your individual rebate code located on your Official Rebate Form.

a) Completed Alcon official rebate form.

b) Submit One (1) original package UPC code from product(s) purchased.

c) Original sales receipt indicating total purchase quantity.

d) Copy of original fitting and/or eye exam receipt with date(s) circled.

4·If you don't have access to a computer, call 1-855-785-6629 to obtain your rebate. This will add two weeks to processing time.



Save your boxes and all receipts for rebate submission.

Terms and Conditions. Offer valid only on eligible purchases made between January 1, 2015 and July 31, 2015. Product purchases must be made within 90 days of contact lens fitting/exam. Rebate submission must be mailed in a stamped envelope to the address listed on the Alcon Official Rebate Form and received at this address within 60 days of your contact lens purchase. Only one rebate submission per envelope. Only one rebate submission per envelope. Offer is limited to one fit rebate with annual supply quantity purchase. Offer is not valid in combination with any other offer or rebate. Allow 6-8 weeks for delivery. No P.O. Boxes (except for residents of North Dakota and where required by law), only street and rural addresses are acceptable. Fraudulent submissions could result in prosecution. Alcon is not responsible for lost, late, damaged, illegible, undelivered, or postage-due rebate requests, which will not be honored. Offer valid only for purchases made in the U.S. by U.S. residents. NOTICE TO CUSTOMERS: If you are personally filing a claim for reimbursement from a third-party payer (e.g., insurance company, employer group, etc.) for the purchase of this product, your claim must be based upon your payment less the amount of this rebate. If your doctor is filing the claim, you must notify the doctor’s office of the need to deduct this rebate amount from the purchase price used in calculating the claim.
